The form used on the Class of 1991 site was excellent, so I basically revised what they had there.  Credit to the web mistress of the CHS Class of 1991 for the basic design.

The information submitted will be used in two ways.  First, certain fields will be used for the site for reference for other class mates, while some information will only be on the site if so chosen.  Information submitted that is not included on the site will be included in a pamphlet which will be mailed to all of the class members that fill out this form and include an address.  Please fill out only what you feel comfortable sharing with others in the class.  Thank you!
